How the schedule has changed

At a glance: born in which decade?

Disease doses: each disease a vaccine covers counts as one dose, so a 6-in-1 counts as 6, MMR as 3 and MMRV as 4. Injections are the number of shots. See Definitions.

Guide's own arithmetic, using disease doses (each disease covered counts as 1 dose), injections and diseases from the counts in the tables below. Each decade is represented by one schedule row: UK 1980s = 1983 (reconstruction), 1990s = 1992, 2000s = September 2006, 2010s = 2010, 2020s = January 2020; US 1980s = 1983, 1990s = 1994, 2000s = 2000, 2010s = 2010, 2020s = 2020. 'Born in 2026' = UK schedule from 1 September 2026; US January 2025 schedule (the court restored 'the version as of May 2025', which has the same doses apart from COVID-19). A child's actual schedule depended on the year of birth within the decade, because schedules changed during each decade. Flu and COVID-19 counts use the assumptions stated under 'How this guide counts'.

With combination vaccines, the January 2025 US schedule needs 21 injections instead of 31 (no flu or COVID). How: Vaxelis at 2, 4 and 6 months (replaces 11 single jabs with 3), Quadracel or Kinrix at 4-6 years (2 jabs to 1), ProQuad at 4-6 years (2 jabs to 1). CDC notes say MMR and varicella should be given separately for dose 1 at 12-47 months unless parents prefer MMRV, so ProQuad is not assumed at 12-15 months. source

In force now (22 Oct 2026): CRS reports that the court order of 16 March 2026 put the US schedule back to "the version as of May 2025" source. That version has the same doses as January 2025 except COVID-19. GAP: the guide could not machine-read the COVID-19 category on CDC's live page; check it there. source

Guide's own total at the routine visits: 48 disease doses and 15 injections (the disease doses include 2 rotavirus doses by mouth), plus about 14 yearly nasal flu sprays (14 more disease doses, no injections).

Babies at higher risk may also be offered hepatitis B at birth and 4 weeks, BCG, or injected flu (selective programmes on the same UKHSA schedule). Brand shown: the 6-in-1 may be Infanrix hexa or Vaxelis; MMRV may be ProQuad or Priorix Tetra.

Consent: the Scottish Government reports that "Vaccinations are not compulsory" source. The UKHSA Green Book reports that, to be valid, consent "must be given freely, voluntarily and without coercion" source.

US (CDC schedule as restored by the March 2026 court order)

source live page

Guide's own total at the routine visits listed: 51 disease doses and 32 injections (counting the RSV antibody, MenB and HPV dose 1 as listed; HPV dose 2 comes 6-12 months later), plus yearly flu and any COVID-19 doses. Combination vaccines cut the injections, not the disease doses; see the US counts above.

Hepatitis B doses 3 and IPV dose 3 may be given at any visit from 6 to 18 months; injection counts assume 6 months. COVID-19: the January 2025 edition recommended it for everyone from 6 months; ACIP moved it to individual decision-making on 19 Sept 2025 source, but the court stayed votes by the newly appointed ACIP members source. GAP: the category now shown for COVID-19 on CDC's live page was not verified by this guide. If the stayed January 2026 schedule takes effect, rotavirus, flu, COVID-19, hepatitis A, hepatitis B (mother negative), MenACWY and the RSV antibody move to high-risk and/or shared decision-making, and HPV becomes 1 dose source.

School exemptions by state: NCSL reports that all 50 states and DC require certain vaccines for school and all allow medical exemptions. 29 states and DC allow religious exemptions only; 16 states allow religious or personal-belief exemptions; Minnesota allows personal (conscientious) exemptions; 4 states (California, Connecticut, Maine, New York) allow no non-medical exemption source.

State-by-state exemption rules (NCSL, updated 1 Sept 2026)

Religious exemption only (29 states + DC): Alabama, Alaska, Delaware, District of Columbia, Florida, Georgia, Hawaii, Illinois, Indiana, Iowa, Kansas, Kentucky, Maryland, Massachusetts, Mississippi, Missouri, Montana, Nebraska, Nevada, New Hampshire, New Jersey, New Mexico, North Carolina, Rhode Island, South Carolina, South Dakota, Tennessee, Vermont, Virginia, Wyoming. DC and Virginia also allow a personal exemption for HPV only.

Religious or personal-belief exemption (16): Arizona, Arkansas, Colorado, Idaho, Louisiana, Michigan, North Dakota, Ohio, Oklahoma, Oregon, Pennsylvania, Texas, Utah, Washington, West Virginia, Wisconsin. In Louisiana an exemption is by written dissent, with no reason in statute. In West Virginia exemptions are on hold while the state Supreme Court considers the case. Washington removed the personal-belief exemption for MMR in 2019.

Personal only: Minnesota (notarised statement). No non-medical exemption: California (removed 2015), Connecticut (2021), Maine (2019), New York (2019).
